I hereby authorise my child to attend the above event and I am aware that my child will participate in both outdoors and indoors activities. I consent for my child to be photographed and that my child’s photographs should be used on the group’s (church) website, forum and for advertising (marketing) and other purposes relating to future BSS events.
I declare that I have been informed that my child’s photographs will not be used for purposes other than that stated above.
